PCOS - Polycystic Ovary Syndrome Treatment

Polycystic ovary syndrome (PCOS) is a common endocrine disorder among women of childbearing age group with no predilection for race. 1. PCOS diagnosis can be reached if 2 of the 3 criteria are present:

  • Oligo-ovulation (fewer than 8 menses per 12 month period)
  • Clinical hyperandrogenism and/or biochemical signs of hyperandrogenism
  • Polycystic ovaries (>12 follicles in each ovary measuring 2-9 mm in diameter and/or increased ovarian volume >10ml) by ultrasonography

PCOS SYMPTOMS:

  • Obesity: Losing 5% to 10% of body weight may help making periods regular. It also helps in managing blood sugar levels and ovulation.
  • Menstrual Irregularities - Oligomenorrhoea (57%), Amenorrhoea (8%) Weight gain
  • Infertility is one of the most difficult symptom. It can lead to frustration and resentment that can affect relationships with your partners.
  • Skin manifestations: Acne, Hirsutism, Seborrhoea, Androgenetic Alopecia, Acanthosis Nigricans, Striae and Acrochordons
  • Anovulation: A leading cause of infertility; lack of ovulation is one of the reasons many women with PCOS have difficulty in conceiving.
  • High cholesterol & high blood pressure: Women with PCOS are at a higher risk of comportised cardiovascular health. These symptoms are worrisome and must be monitored regularly.
  • Sleep disorders: Sleep apnea and insomnia are the most prevalent of sleep disorders reported in women with PCOS.

PREVENTION:

  • Eat a nutritious diet that is high in fibre and low in simple carbohydrates and sugars. Carbohydrates should be the ones with low glycemic index.
  • Establish and maintain a healthy workout routine - physical fitness will keep all hormonal imbalances under control.
  • Incorporate nutritional supplements into a healthy diet.
  • Lifestyle modifications: Don't skip breakfast and sleep early so as to avoid imbalances in hormonal rhythm.

PCOS TREATMENT:

  1. Behavioural strategies: Less usage of mobile, ipads etc.
  2. Dietary interventions: Take a balanced diet. For weight loss, energy deficit of 30% or 500-750 kcal/ every day (1200 — 1500 kcal/d) is required.
  3. Exercise intervention:

For preventing weight gain and maintaining good health:

  • Muscle and bone strengthening exercises
  • Adults: 150min/week moderate intensity or 75 min/week vigorous physical activity
  • Adolescents: 60min moderate to vigorous activity/day

For weight loss:

  • 250 min/week moderate intensity
  • 150/min/week vigorous intensity
  • Minimize sedentary/sitting/screen time
  • Daily 10000 steps ideal
  • 30 minutes of structured physical activity or 8000 steps

ORAL MEDICATIONS:

COCPs (Combination oral contractive pills), anti obesity drugs, inositol etc.
